Proxy Access to medical records

Proxy access allows someone other than the patient to access and manage parts of their GP online services account. The proxy is given their own online access account (rather than using the patient's login details). Examples of when this may be appropriate include:

  • The parent of a child under 11 years of age
  • The family member/appointed person with Lasting Power of Attorney for the heath of a patient who lacks mental capacity, for example, through mental or learning disability or dementia
  • Care home staff to enable repeat prescription ordering for their residents

To set up proxy access for a patient, please send your request in writing or by email to nnicb-nn.c84085@nhs.net, including:

  • The patient's name, NHS number (if known), date of birth, address, telephone number and email address if known
  • The proposed proxy's name, telephone number and email address
  • The reason for requesting proxy access
  • What aspects of the medical record proxy access is being requested for
  • Whether the patient has consented to proxy access

The practice will then consider the request and may contact the patient to discuss it with them as appropriate.
